02 Next 2010 HOW all, Can anyone please give me the format of " declare for not deducting PF" to be collected from the employees ( Employees joined with a basic salary out 12200 p.m. or not opting for PF.)( no previous employment)

“I want to inform you that I was not a member of EPF Scheme anywhere once enter this establishment. As my basic+DA wages can above statutory earned limit of Rs.6500/- for computational P FLUORINE, I am not leaning to become EPF member at present. IODIN am aware of the consequences in giving this declaration.”

This can be modified depending to the our insurance.
